HospiceHospice and Long Term Residential CareAdult ServicesThe Loudoun County Department of Family Services conducts pre-admission screenings for nursing home and adult care residence placements, and in-home Medicaid personal care services, home-based care services (Companion and Homemaker Services), case management, counseling, advocacy, and information and referral services.
Capital HospiceCapital Hospice serves persons of all ages with such diseases as cancer, Alzheimer's dementia, heart, lung or kidney disease, AIDS and ALS (Lou Gehrig's disease), and whose prognosis is estimated at less than one year. The hospice provides medical care by a team of doctors, nurses, chaplains, social workers, home health aides and volunteers, in the patient's home, nursing home or assisted living facility. Grief counseling is offered to the family. Capital Hospice operates an inpatient facility in Arlington. The hospice also contracts beds for acute care at Loudoun Hospital Center. Loudoun Nursing and Rehabilitation CenterThe Loudoun Nursing and Rehabilitation Center provides residential nursing care and assists residents with daily living activities. Heritage Hall Nursing and Rehabilitation CenterThis 164-bed long-term care facility provides intermediate and rehabilitative care. The facility provides a daily activity program. Physical and speech therapy is also offered.
Johnson Center at Falcons LandingThis 60-bed long-term care facility provides intermediate and rehabilitative care. Physical, speech, occupational, IV and respiratory therapies are offered. Limited respite services are available. Medicare, third-party insurance and private pay.
Northern Virginia Long-Term Care Ombudsman ProgramThis free service assists the consumer in making an informed decision about long-term care providers such as nursing homes, board and care facilities, and homes for adults.
